This request is intended for students and must be submitted while logged into their CCSD student account. If you receive the message "You need access," it means the student is not currently logged into their CCSD account in that web browser. Please log into your CCSD Gmail account and try accessing the Form again.
New or Returning to CCSD this school year? If you have not yet received access to your CCSD account, you may email your school counselor with your request instead. Please be sure to mention that you are new to CCSD and do not yet have access to your student account.
